I paid for a facility for my daughter with mental health care issues for 3 months and the last month the provider moved my daughter against her will to a low-level facility causing her extreme emotional trauma resulting in a -. I paid $5500.00 for the month and the provider paid $2000.00 for the unauthorized house they moved my daughter to. I am asking to be refunded the last month. I DID NOT RECEIVE THE SERVICES I PAID FOR-
