Medical Cont'd attempts collect debt not owed Debt was paid New Jersey

I ran a credit report through - on -/-/-. To my surprise, I found I had a balance due from - accounts from Quality Asset Recovery ( QAR ). On -/-/- I had received a letter from QAR stating my balance was $0.00 on - accounts as noted in attached documentation. Around the end of - -, I had called QAR to arrange a payment plan for my debt of $2500.00. Myself QAR agreed to make - ( - ) $620.00 payments ( totaling $1800.00 ) as you will see in attached checking account statements provided. I have not notified QAR as to my findings and I am asking for your help to resolve my concern with my credit report stating I still have a balance with QAR of $1400.00 ( $310.00 for - account and $1100.00 for another account ).
