I had previously submitted Complaint # -. I am an - year old -. I sent in NEW information in this complaint after Chase Bank official - -, Cardmember Services responded - -, 2016 advising MERCHANT ( - - - , - ) responded. This new information is of great concern as the " Hold '' I put on my Chase Bank Credit Card of $5400.00 + $500.00 is about to be removed and given to this unscrupulous - - -, who not only improperly -, after written agreement and - promising No Posts would enter - -Wrong! They did - causing -. He has also refused my rightful, paid-for - discount.


- - also refused the - - - of - % Discount, which I paid for, and of which he is a member, and how I actually found him. There is a - - letter I have attached where they claim - - - will not even speak to them, and advise I report him to the Florida - - -. I have already reported - - - to the Florida Heath Dept. who has contacted me and are about to deal with him for his unprofessional behavior. What is at stake here is my $5400.00 + $500.00, total : $5900.00. 1 ) I am enclosing the Letter from Chase Bank Official, - -, Cardmember Services who demands : I Provide a - Opinion from a reputable Merchant or Service Provider if work by - - - was done poorly or was unnecessary. Also demands specific information showing Why the work was poor or unnecessary. Demands an Invoice showing what - Merchant would charge to resolve the issue. The - Opinion should be written on Company Letterhead signed and dated by the Merchant!! These demands are unconscionable! No Doctor voluntarily demeans the work of another doctor! This is a known fact! What is being demanded here by Chase, or through -, is tantamount to the level of information a - suit in court would require. It demands an Expert Witnesses Doctor who is paid for his opinion. 2 ) I am including documents which - - - had me sign before -. If you read them, they are also unconscionable : To force a patient to sign, before - that : " I agree not to file/engage in any Credit Card or/Care credit chargeback as procedures chargebacks are not permitted with this office under any circumstances. I will not engage in any disputes through Credit Card Co. or - - - - to recoup fees on any medical and all procedures provided. This says : " No Matter how I harm you, you agree to do nothing ''. Frightening that a Licensed - - would even request a patient to sign this! Note : I already submitted in Complaint # - Notes and - from - - - -, which I am attaching here. I am attaching New Information -/-/2016 from - - - - - who found - by - -. Recommends - - PLEASE NOTE : - - - before - are attached ; where he assured me and my husband NO - would enter my - -.
