Identity theft / Fraud / Embezzlement California

I 'm a victim of identity theft. US Bank - is reporting a credit card that is not mine. I 've disputed this account and it has not been deleted off my credit. I have a police report and identity theft affidavit that I will include. Please delete this account off my credit file immediately.
