I am forwarding to Chex Systems the police report. I am disputing - items on my credit report with them. I have submitted a FTC affidavit and they refused to take because of no signature by an officer but I was able to get it notarized and the items I am disputing and should be removed due to Identity theft is as follows : - - - - - - - - - - - - - - - - - - -
