Medical Disclosure verification of debt Right to dispute notice not received California

I am a - year old college student .Just recently I found out that I have an unpaid debt ( $450.00 ) in my credit report from -/-/- which I never knew about. Upon further investigation I found out about an unpaid medical bill my insurance company has refused to pay. Now, I have never received a single correspondence from the hospital nor from the collection agency.The only reason I found out about this debt now in -/-/- is because I applied for credit card and was denied due to the above mentioned debt .I am devastated because I pay all my bills on time. I do n't have any credit card debt since I pay them fully at the end of each month. I feel my rights have been violated and neglected and I have been put in this situation unfairly.
