PDQ Services, Inc. Debt collection disclosure verification of debt right to dispute notice not received December 25, 2015 - I went to the doctor for a medical condition with -. I completed all of the necessary paperwork and the doctor 's office filed a claim on my behalf which was supposed to have been sent to the insurance company ( - ). I never heard anything back from the doctor 's office after that nor did I get any information from the insurance company stating that I owed any money. Shortly after that visit I moved to a new address and never received a bill from the insurance company or the doctor. About one year later, I was checking my credit reports and I find - entries of collection on my reports... |
