I was admitted into the hospital there and treated but I 'm on medicaid out of - this should of been all covered. I contacted this collection agency and informed them of this.I 've been medically covered every since - of 2005. I 'm at a loss as to what they want me to do. This was originally a - automobile club collection which i have never had -. I disputed it through Experian - - in - of 2014 and again in - of 2014. I had dental insurance and always paid my coinsurance amount prior to leaving the office. I called the -, FL office where I was a patient, and I was told it was a clerical error on their part. The employee said someone entered a wrong code into their computer system and the - office, the only - - office I have ever been to, was not listed as my primary or home office. I disputed this with the credit bureaus and AWA responded leaving it on the file and sending me a copy of the promissory note ''. The signature on the note is a clear forgery. I have attached a copy of this note along with a copy of my signature on a recent agreement. They use a small l '' in my last name..I ALWAYS write my name capitalizing the L '' in - -.
