Incorrect information on credit report Personal information Texas

I believe my ex wife is not reporting my address/phone number to the pediatric doctor 's office our children go to. About two years ago, I discovered 3 charge off for medical bills from - in -, -. The collection agency 's name is Consolidated Recovery Systems which also is in -. After reviewing my report, I contacted - and advised them I did not receive a phone call nor any bills from them. The call taker advised my ex-wife had told them she did not have any contact information on me in which she does. I also did not receive any phone call or written notification from consolidated recovery regarding the charge offs. Now I have the same problem again where I just pulled my credit report and found a $91.00 charge off from - from -/-/- and it was updated on my credit report by consolidated recovery on -/-/-. My ex-wife has my address and phone number and when I last spoke to -, my number was the same as it is today. I relocated to -, - in -/-/- and I have not received a phone call nor a letter from - or consolidated recovery regarding this adverse account. Please help because I do n't understand how I 'm constantly receiving charge offs without notification and it 's damaging my credit. Had I not pulled my credit, I would n't have known about the charge off. It seems to me, there are no thorough attempts to contact consumers from consolidated recovery before they report charge offs. With technology today, It is not hard to locate anyone so I do n't understand why I 'm not being contacted. Whether my ex-wife is intentionally not reporting my address ( responsible party ) or the pediatric office fails to make attempts, I would expect consolidated recovery systems to do their due diligence before placing a charge offs on my report.
