Incorrect information on credit report Information is not mine Utah

I recently applied for a credit card through -. I was not approved. I was sent a letter stating that there is a fraud alert on my credit file, and no telephone number for the lender to contact me. I have not initiated a fraud alert, and I do not understand why my telephone number is not available for a lender to contact me.
