Medical Disclosure verification of debt Right to dispute notice not received Georgia

I have no idea where this debt came from. The collection agency has not mailed me enough information to verify that the debt belongs to me even though I mailed them a certified request. They only mailed me a letter giving me the name of the hospital and amount. They refuse to provide me legitimate proof that this is my bill such as the services provided, the date of service or my signature. I also never given a chance to dispute the debt. I never received letter or phone calls in reference to this debt.
