Balance transfer California

I received a 0 % interest for balance transfer from CITI. After filling up the application and being approved, I proceed to do the balance transfer. Once the balance transfer was completed I received a letter stating the terms of the transfer including a 22.49 % interest fee. I requested a revision of the account, requested evidence of my acceptance of the offer with an interest rate, and CITI is not willing to provide the information.
