-/-/- To : - Dispute Department Subject : Unauthorized Account Opened Citibank Credit # - ( last four digits ) Name : - SS # : - Contact : Cell phone # : - Email : - To Whom It May Concern : Much to my shock, I learned today that an unauthorized Citibank credit card account ( # - ) has been opened in my name ; this unauthorized account currently has an outstanding balance of $290.00. I am writing to let you know that I did not solicit this credit card account from your bank ; I did not authorize this account to be opened ; and, I was not notified that this account was opened. I am very upset that your bank would do such a thing without my permission. During a conversation your customer support team this morning, I was informed that your bank opened this account on -/-/- as a result of a transitioning me as a Costco member from an - credit card over to Citibank card. I suspect that you then debited this unauthorized Citibank account


Unbeknownst to me, I had this obligation over the past seven months. Moreover, it has accrued late fees ( and, or, penalties ) over the subsequent months. What is particularly upsetting to me is the damage that you 've done to my credit score by reporting my non-payment of this unknown credit to the agencies. Consequently, my credit score has been severely damaged. Prior to this incident, my credit score was ~ - ; now, it hovers at -. Please note : I have been using Citibank credit responsibly since -/-/-. Over the years, I have demonstrated my willingness and ability to make monthly payments on a timely basis. I have never once been late for I have always scheduled payment in advance of the due date. Moreover, I paid off one card ( account # unknown ) on, or around, -/-/- with a final payment of $4900.00 ; I have paid another account ( # - ) on, or around, -/-/- with a final payment of $8100.00. To my knowledge, all my accounts with Citibank were closed as of, approximately, -/-/-. In my telephone conversation with Citibank customer support today, I have requested that all - monthly statements for this unauthorized account ( - ) be sent to me. I will assume for now that the only charge on this unauthorized account is for my Costco membership and accrued late fees. Let 's hope that this is the case, and this card has n't fallen into someone else 's hands. Once you 've reviewed my credit history and established that I am a responsible borrower, I would appreciate if you would do the following : 1. Reverse all late charges and, or, penalties on this account 2. Inform me of the $ amount owed for the Costco membership ; I will arrange to pay this amount immediately Furthermore, I would like your bank to take the following remedial action to restore my good name with the credit reporting agencies : 1. Contact all credit reporting agencies and request all Citibank reported late payments and/or penalties associated with this account be reversed. 2. Furnish me proof that such action has been taken with the - major agencies. In conclusion, I would appreciate your prompt action to undo the harm this done to credit, and to restore my faith in your bank. Sincerely, - - -
