Credit card Cont'd attempts collect debt not owed Debt is not mine California

Hello I 'm a victim of fraud and stolen identity of credit card. I have been notified by the collect - agency? - stating that I have a credit card that has been open since estimate on -/-/- and last payment received on -/-/- total payment of $200.00. I have neither paid this amount nor did I open an credit card with this company. They are trying to garnish from my paycheck/ salary at this time, and I have contacted this company and I have asked for original credit card application with my signature they promised to send, but they did not send it. They have only sent me a copy of credit card statement with address which I do not know of. Please help!
