Medical Cont'd attempts collect debt not owed Debt was paid Pennsylvania

I had medical assistance at the time of the - visit, all referrals were in order and insurance card was given. No bill was asked upon leaving the facility, no bills were sent in the mail, about a year later I received a collection notice for a debt owed on that specific date of service. How can you send something to collections that you never gave the person a chance to pay, I did n't even know I owed anything. But anyways, I then called the collection agency and ended up paying the bill they asked. I thought everything was fine.. Well about 2 years after I received a new notice from another collection agency for the same date of service saying I owed them money again. How can this be?
