Payroll card Unauthorized transactions/trans. issues Michigan

I went to the bank on -/-/16 to do a cash advance for - from my Rushcard debit card and the teller never received a authorization number from the terminal or printout to give me my money. The teller stated it was not going through, it kept giving him a error message.But the withdrawal and fee was pending on my account till this afternoon on -/-/16. It cleared my account and now Rushcard Does not want to return my money when they were having system issues and doing a system maintenance update the same day. I am very unhappy, because it was not my problem and customer service is not able to see the error on their end.
