Credit monitoring or identity protection Problem with fraud alerts Arizona

I have been a victim of identity theft since -/-/2012. I have an extended fraud alert on my credit file. Recently, there has been an increase in attempts by identity thieves to open credit/loan accounts using my identity. Most of the creditors will call by phone and ask me to identify myself and ask if I applied for credit with them. When I deny that it was me who made the application, they simply deny the application and send me a letter in the US Mail stating that they were unable to approve the application because they were unable to verify the information that was provided on the application. Some creditors will send a second letter stating that they will submit a request to delete the inquiry to the credit reporting agencies. My complaint is two-sided - one side is with a particular company that calls and asks for all of my identification ( name, address, phone number, date of birth and social security number ) when notifying me that an application for credit has been made with them in my name. It makes me very uncomfortable when I am asked for this information over the phone and I am told that the company can not verify my application without this information. I am not confident that this request for my information is even legitimate because my information is not requested by all of the companies that notify me about applications that they have received in my name. The other side of my complaint is with a particular credit reporting agency which, although there is a fraud alert on my credit file, has released my credit report to that company.
