Huntington Bank is refusing to produce the records requested by a subpoena dated - -, 2015, for copies of specific documents from an IRA account for a deceased account holder. It appears Huntington Bank issued checks to the wrong beneficiaries on the IRA account of concern. Huntington Bank personnel indicated they disbursed beneficiary checks to - ( - ) individuals and they provided the names of those individuals ( - sisters and - nieces of the deceased account holder ). A Huntington Bank document in my possession indicates there was only - ( - ) primary beneficiary ( - of the sisters of the deceased ) and - secondary beneficiaries listed on the beneficiary document. The beneficiary document indicates that secondary beneficiaries are not supposed to inherit any monies unless all primary beneficiaries are deceased. The primary beneficiary is alive. On the document we possess it is noted that the primary beneficiary is supposed to receive 25 % of the account funds. Therefore, the remaining 75 % of the account funds are supposed to be probated ( or distributed via a settlement between the parties ). I am a probate beneficiary and I also am acting on behalf of the


causing a serious delay in the resolution of this probate estate matter and further duress by this delay. Huntington Bank needs to provide copies of actual requested documents to our attorney immediately. If Huntington Bank is not able to provide copies of actual documents matching the distribution made by their agents, Huntington Bank needs to request that their insurance company restore the funds to the IRA account and await further instructions from our attorney regarding the proper distribution of the beneficiary funds. Also, Huntington Bank billed our attorney $16.00 for sending our attorney a letter with the information improperly provided ; the subpoena requested copies of actual documents and not someone 's typed interpretation of those documents ; Huntington Bank needs to reimburse me for that charge ( passed on to me by our attorney ). Huntington Bank will also need to reimburse me for all attorney fees, legal fees, expenses and court costs created by their mistakes and improper delay of this case. Thank you for your assistance.
