Medical Disclosure verification of debt Not disclosed as an attempt to collect Indiana

I was given noticed that HELVEY & ASSOCIATES, INC. has taken my debt, and I was given 30 days to dispute it, but I have not been able to get in touch with them as their phone number is disconnected. The debt also is not valid as my insurance company was not notified of the claim and I was told that no one contacted them about my medical bills or hospital fees and they have requested to see the claims from the hospital.
