I was a patient at - Hospital on -. I did not have a solid place to live when discharged and used a mailing address for my mail. When discharged on - I went to live with a friend and had my mail forwarded from the PO Box to a physical address. It was a temporary forward because the location to my mail box and the location of the apartment were not close and I did not know the length of time I would reside there. In - I recieved an alert that a collection appeared on my credit report. I called in - to see what was going on and it was for the co payment for services I received in -. I informed - that I never recieved a dunning notice or anytime of notice from there company that there was a balance.
