On -/-/14 I underwent a - procedure ( - ). Prior to the procedure I was assured by - that my insurer, -, would cover all expenses, including -, as long as the - bill was no more than $400.00. I was further assured that this had not been an issue in prior procedures. Base on that information, I anticipated that my - bill would be covered. Since that time, I have received constant demands for payment of $2200.00 from - - - -. I had no choice in the selection of this -, who is an out-of-network provider.
