I sent - - at - - Hospital in -, NC copies of my daughter 's Medicaid card for her - -/-/-. All the other providers ( aka, the surgeon 's bill ) was covered by Medicaid. Why - - purposely keeps losing my daughter 's Medicaid information is beyond me.
