Identity theft / Fraud / Embezzlement Washington

-/-/2015, a credit card was opened with this lender using my social security number and date of birth under a different name that is not mine nor a name that I have ever gone by. I found out this past week that I became a victim of identity theft and have since contacted this lender and -, - and - to flag my credit file for fraud. I have never had an account with this lender.
