I have a credit card with Fifth Third bank. On -/-/2015 I was billed - for next years annual fee. I received the bill on -/-/2015 and called the customer service line at - and told them that I wanted to cancelled card as I did not want to pay a - annual fee for a credit card. They closed the account but are refusing to reverse the unauthorized annual fee.
