Unable to get credit report/credit score Problem getting report or credit score Ohio

On - -, 2015, I received an adverse credit letter from -. They stated their decision was based on information received from - credit reporting agencies, - and IDA , Inc . The information was inaccurate. On - -, 2015, I requested my credit reports and credit scores from - and SageStream ( formerly IDA ). I promptly received my Credit Report and Score from - ; - ; -. The credit report from - revealed no adverse information. Instead of sending my Credit Report and Score, SageStream sent me a letter requesting identify information as well as - verification documents. This was done along with my second request for my Credit Report and Score on -/-/2015. To date I have not received a response from SageStream and I am continually being assessed the - monthly bad credit fee.
